Form W-9 (Request for Taxpayer Identification Number and Certification) is provided by individuals and businesses to clients who will report payments to them on a 1099. Provides: name, business name (if applicable), tax classification (individual, S-corp, LLC, etc.), TIN/SSN, address. Self-employed individuals typically complete W-9s before receiving the first payment from a new client. Clients use the W-9 information to prepare year-end 1099s. Don't sign blank W-9s. If a client requests a W-9 for a transaction that shouldn't be 1099-reportable (purely product sales under $600), ask why before complying. Submit via secure channels — W-9s contain sensitive identity information.
