A copay (copayment) is a flat dollar amount you pay for a specific healthcare service or prescription, regardless of total cost. Common copays: $20-$40 for primary care visits, $50-$100 for specialists, $250+ for ER visits, $10-$50 for prescriptions. Copays apply after deductibles in some plans, before in others, depending on policy design. Copays don't usually count toward deductibles but do count toward your annual out-of-pocket maximum. Plans with low copays and high premiums favor frequent healthcare users; high-deductible plans with HSAs favor healthy households who can self-fund routine care.
